Make a Referral

We welcome referrals for adults with intellectual and developmental disabilities who may benefit from Personal Supports or Respite services.

Optima Family Services accepts referrals from Coordinators of Community Services, Support Coordinators, Social Workers, Case Managers, Hospital Discharge Planners, families, guardians, and community partners.

Our team will review referral information and follow up to determine whether Optima can meet the participant's needs, staffing requirements, service preferences, and authorized service plan.
